type over an existing example

• Your Cover Sheet Should Include:

  • Name, address & phone no. of company
  • Name, title, address, phone no. of
    • owners/corporate officers
  • Month and year your plan was prepared
  • Name of person(s) who prepared the plan
  • Copy number of plan
